PAR-Q and Waiver Form

As your personal trainer for the DR fitness program, I want to make sure that you approach exercise as safely as possible, hence the two parts of this form. I use this form with all of my clients, if you have any questions at all please feel free to ask.

  1. PAR-Q

This form acts as a PAR-Q (Physical Activity Readiness Questionnaire) that will help determine whether or not it's safe for you to begin a new exercise program. Please answer the following questions honestly, and provide as much detail as possible where applicable.

2 Wavier

It is important that you understand that physical exercise can be strenuous and subject to risk of injury. You are urged to obtain a physical examination from a doctor before participating in any exercise, including those provided by Dylan Russell Fitness.

You agree that if you engage in any physical exercise or activity, you do so entirely at your own risk. This waiver and release of liability includes, without limitations, all injuries which may occur as a result of: (a) your participation in any activity or participation in any exercise routine and (b) instruction, training, supervision, or dietary recommendations by Dylan Russell Fitness.

You acknowledge that you have carefully read this "waiver and release" and fully understand that it is a release of liability. You expressly agree to release and discharge Dylan Russell Fitness from any and all claims or causes of action and you agree to voluntarily give up or waive any right that you may otherwise have to bring a legal action against Dylan Russell Fitness for personal injury or property damage. By signing this release, you acknowledge and understand its content and that this release can not be modified orally.
